NEW PATIENTS

Thank you for choosing Allergy and Asthma Specialists of South Florida, LLC for your family's allergy care.  To ensure a smooth initial visit:

  • Dr. Brodtman is an Immunologist who cares for Immunodeficient patients who are very susceptible to life threatening infections. In addition he is a strong advocate for immunization.
    ​*Please be advised our office policy won't allow us to establish with patients who do not immunize
  • Please download and complete the new patient form listed below and bring them to your visit, (If you are unable to complete the forms prior to your visit please arrive 15 minutes early to complete registration process at the office).
  • New patient appointments are typically two hours in duration. Please plan accordingly.
  • Please bring your insurance card if you will be utilizing an insurance plan.
  • It is the patients responsibility to have any necessary referrals at the time of the visit.
  • Photo ID is required at check in
  • Please bring a list of any medications, vitamins or supplements including the dosing information.
Where appropriate, allergy testing may be performed during the first visit. New patients are advised to avoid all antihistamines both oral and nasal, for a minimum of 48 hours before the appointment. Skin testing is typically done on the back so consider this when dressing for the appointment.

We encourage open communication and will directly advise you about your symptoms, diagnoses and recommended treatment plans.  Please feel free to ask questions you may have before, during and after your treatment is discussed.
